CBA Blue cards (for the new MIIS Medical and Dental benefits that take effect January 1) were mailed 12/14 to benefits eligible employees. On 12/9, Flex Spending cards, which look like credit cards, were mailed to program participants. Both cards were mailed to the home address that MIIS HR had on file at the time of the mailing.

The group number for employees is 50643. All MIIS employee info is loaded into CBA’s system so employees can go on-line to www.cbabluevt.com and register as new users. Once on CBA Blue’s website you will be able to get your own member number. Or you could call customer service at 1-888-222-9206 and the rep will be able to give you your member number.

If you need to access medical services out of the country, you should provide your insurance information, but will probably have to pay for the medical expenses yourself and then when you get back to the US, submit these expenses for reimbursement. You should get an itemized receipt for the services so that CBA can process the claim. This type of reimbursements should be coordinated through Cathy Vincent.
